CONSENT FORM

Yeshuah Ministries International produces a range of communications resources to communicate the work our church does, for example forthcoming events through various communication channels like our website, for example. We like to share the experiences of real people who are using our services, whether these are our church members or not in our communications as it helps to demonstrate the difference our work is making and the inclusive nature of people participating. By completing this form, you give us permission to use your images, whether still or otherwise, in our communications for the next three years. Your assistance is much appreciated

Social media: Yeshuah Ministries International social media pages, e.g. facebook, instagram etc

Publications: Yeshuah Ministries International leaflets, posters, newsletters and other such materials

Can I remain anonymous?
You can choose to have your real name published or remain anonymous (in which case, we will use a false name). Please tick one of the following options:

Are there any identifying features you do NOT want included in our recordings, images etc? For example, your location or the age of your children

Your images, voice recordings, comments, etc will not be used or stored for any longer than six (6) years from the date of your consent, unless you ask us to stop using them before then.

If you are under 18, we need written permission from a parent, guardian or your responsible adult.
